A single patient with refractory cluster headache, whose attacks persisted despite conventional preventive treatments and only responded to high-dose glucocorticoids, experienced complete suppression of nocturnal headache attacks and rapid relief from insomnia when treated with ramelteon. This is the first English case report describing ramelteon’s preventive efficacy in cluster headache.
